It doesn't have to be complicated. It's simply a folder with a list of all your important information and copies of important documentation.
I recommend dividing your documents up into categories.

Social Security Cards
Copy of recent credit report. You can get a free report every year.
Most Recent Tax Return
Bank Account(s) Information
Financial Portfolio Account(s) Information
Credit Card Account Information
Mortgage (or rental agreement)
Utility companies and account information
Copy of your home warranty policy if you have one
Copy of homeowners Insurance
Other Account Information
Credit Card Accounts
Vehicle Loans or Leases
Vital health information for each family member, including blood type, allergies, conditions
Short- and Long-Term Disability Insurance
Last Will & Testament
Health Care Power of Attorney
